Getting Started

To get started with your PHR, we recommend starting with your most recent information first. The HealthManager has a QuickStart wizard assistant that will walk you through entering this basic information, and results in a printable wallet emergency card.

To begin gathering and entering your information, follow the 5 steps below:

STEP 1: Contact your past providers offices or facilities to request copies of your records. There are several questions you will want to ask. You will want to know how your provider keeps the records - if they are kept electronically, you may be able to access them directly, otherwise you will have to get copies. You may want to ask for guidance on which parts of your records you will need. You can contact insurance companies to gather records as well.

STEP 2: You will need to request an Authorization for the Release of Information form. Complete this form and return it to your provider. Many providers may charge a fee for copies of your records - to cover the costs of copying and sending them to you when necessary.

STEP 3: Once you have your records, you can begin to enter your information into your PHR. The Metavante HealthManager allows you to store information for each of your family members. You should also update your PHR after any visit to a care provider. Click here to download a form to take to your provider’s office with you.

STEP 4: Set a password to protect your information. A user will be required to enter a password to access all of your other information.

STEP 5: Store your information to your computer. HealthManager saves information as you enter it, but it is important to make sure all of your data is backed up on your hard drive. HealthManager will prompt a back-up after every five accesses.

Communicating your PHR with your provider

Bring your PHR with you to all of your visits. This can be done though printed reports from the desktop version of the Metavante HealthManager. And if your provider has Internet access, you can log-on to your PHR to share your health information electronically. Regardless of which method you choose, be sure to generate an Emergency Medical Card to carry with you in your wallet. You may not always have your PHR with you, and it is important that your emergency information be with you at all times.
